Where to send completed
claim forms for students who are on the Reed insurance plan

In order that benefits may be applied accurately, students should send all completed claim forms to:

Health Insurance Waiver Form

Any student who has not turned in a Health Insurance Waiver Form (pdf) for each school year will be enrolled in and charged for the College's health insurance policy. The Waiver must be received in the Business Office by the first day of classes.

Student Health Insurance Plan

Our Student Health Insurance Plan for the 2007–08 Academic Year is provided by The Chickering Group, an Aetna company in partnership with USI Northwest. The policy is a comprehensive medical plan. A Schedule of Benefits is included in the mailer sent to all matriculating and continuing students in May. View a brochure (pdf). You may also find a directory of participating Aetna physicians, specialists, hospitals and pharmacies at the Chickering website for Reed students.

International Students
